Person-centred support for different needs

Every person is different. Age, diagnosis or disability alone does not define the care and support someone may need.

Hollborn Care Services supports adults with a range of needs through our Homecare and Supported Living services. We take time to understand each person's abilities, preferences, routines, communication needs, relationships, identified risks and desired outcomes before agreeing how support should be provided.

Our approach is designed to promote dignity, choice, independence, safety and wellbeing while ensuring that care remains centred on the individual.

CARING FOR ADULTS OVER 65 YRS

Support that respects independence, routines and choice

We support adults aged 65 and over who may require assistance to continue living safely and as independently as possible.

Support is based on individual assessment and may include assistance with personal care, everyday routines, mobility, nutrition and hydration, medication, companionship and maintaining meaningful activities where these form part of the agreed care plan.

We recognise the importance of familiar routines, relationships, culture, personal history and individual preferences. Our aim is to provide the right level of support without unnecessarily taking over tasks a person can safely manage for themselves.

Individual support for younger adults

Adults under 65 may require care or support for many different reasons. We focus on the individual rather than making assumptions based on age or diagnosis.

Depending on assessed needs, support may help with personal care, everyday living, maintaining routines, accessing the community, developing or maintaining independence, relationships, wellbeing and achieving personal outcomes.

Individuals should be involved in decisions about their support and encouraged to exercise choice and control over everyday life wherever possible.

DEMENTIA

Understanding the person, not only the diagnosis

Our dementia support is centred on understanding the person, including their life history, routines, communication, abilities, preferences and what helps them feel comfortable and secure.

Depending on individual needs, support may include personal care, prompts and reassurance, assistance with daily routines, nutrition and hydration, medication support, maintaining meaningful activities and helping the person remain as independent as possible.

We aim to communicate calmly and respectfully, recognise changes in wellbeing or behaviour, and work with families, representatives and relevant professionals where appropriate and with the person's rights, wishes and consent at the centre of decisions.

PHYSICAL DISABILITIES

Support that promotes choice, accessibility and independence

We support people with physical disabilities according to their individual needs, abilities, environment and personal goals.

Support may include personal care, mobility and transfers where appropriately assessed, everyday activities, accessing the community, meal preparation, medication support and maintaining routines and interests.

Our approach is to enable the person to participate as fully as possible in decisions and everyday life, using appropriate risk assessment, equipment and agreed support arrangements where required.

SENSORY IMPAIRMENTS

Accessible support shaped around communication and individual preferences

People with sensory impairments may have different communication, environmental or support requirements. We take time to understand how each person prefers to communicate and what adjustments may help them participate fully in their care.

Support may include assistance with everyday routines, personal care, accessing information, orientation within familiar environments, community activities and communication with others where this forms part of the agreed care plan.

Care staff should follow the person's documented communication preferences and agreed support strategies while promoting choice, dignity and independence.

We begin by understanding the person's circumstances, needs, abilities, preferences and goals.

We consider care needs, communication requirements, routines, identified risks and the outcomes the person wishes to achieve.

Where Hollborn Care Services can safely meet the person's needs, an individualised care plan and relevant risk assessments are agreed.

Support

Care is provided according to the agreed plan, with dignity, choice, consent and independence at its centre.

Review

Support is monitored and reviewed so that care can respond appropriately when needs, risks, preferences or circumstances change.
