Where identified through assessment and care planning, staff can provide respectful assistance with personal routines while protecting dignity, privacy and choice.

Support can include everyday routines such as preparing meals, maintaining the home, organising activities and developing practical skills according to the person’s abilities and goals.

Community & Relationships

Individuals can be supported to maintain relationships, participate in their community and engage in meaningful social, recreational or other agreed activities.

People are encouraged to make decisions about their lives and do as much as they can for themselves, with support provided at the level identified as appropriate.

Enquire

Tell us about the person, their circumstances, current support needs and what they would like to achieve. We can discuss whether our supported living service may be suitable.

Assess & Discuss

We take time to understand needs, abilities, routines, communication requirements, preferences, risks and desired outcomes. The individual is central to this process, with representatives or professionals involved where appropriate.

Plan

Where we can meet the person’s needs, an individualised care and support plan and relevant risk assessments are developed, setting out how support should be delivered and the outcomes the person wishes to work towards.

Support

Appropriately selected staff provide support in line with the agreed plan. We maintain communication and review the service when needs or circumstances change so that support remains appropriate and person-centred.
